The UF Health Orthopaedics and Sports Medicine Institute will be providing Free Pre-Participation / Sports Physicals to all youth, high school, and Pop Warner students that will be participating in athletics for the 2024-2025 school year.
Visit the 2nd Floor of the UF Health Orthopaedics and Sports Medicine Institute on April 22, 2024 from 7pm-9pm.
All forms MUST BE SIGNED by a youth's parent/guardian PRIOR to physicals.
High School Sports:
- Download and complete the Participation Physical (EL02) form from the FHSAA website
Pop Warner Sports:
- Download and complete the Medical/Physical form from the Pop Warner website
