Locations

The UF Orthopaedics and Sports Medicine Insititute (OSMI) treats a diverse nature of conditions. Our primary facility is located in Gainesville, Florida, adjacent to the west side of the University of Florida campus.

To save time, it is recommended to download/complete the patient registration forms and bring them with you on your visit. If you are a referring office requesting consultation or transfer of care, download the consultation or transfer of care forms, and fax with all applicable notes.
