The UF Health Orthopaedics and Sports Medicine Institute will be providing Free Pre-Participation / Sports Physicals to all youth, high school, and Pop Warner students that will be participating in athletics for the 2023-2024 school year.

Visit the 2^nd Floor of the UF Health Orthopaedics and Sports Medicine Institute on April 26^th, 2023 from 7pm-9pm.

All forms MUST BE SIGNED by a youth's parent/guardian PRIOR to physicals.

High School Sports:

• Download and complete the Participation Physical (EL02) form from the FHSAA website

Pop Warner Sports:

• Download and complete the Medical/Physical form from the Pop Warner website
