PDF versionMandatory Requirements for OSMI Volunteers
Everyone must complete the following:
- HIPAA for Researchers (print out certificate)
- Confidentiality Statement (read policy and register, print out certificate)
If you are NOT in one of the categories below:
- College of Medicine (in session) Medical Students
- Site Monitors for approved clinical trials
- UF Students enrolled in (Physician Shadowing) classes (such as MEL4012 or BME6010)
- Sales Representatives fully registered/cleared under RepTrax
- Approved Courtesy appointments
- Approved Internship or Independent Study (Required for Graduation)
- High School Students enrolled in Official Programs (examples-STAR Program, SSTRIDE Program, Health Care Summer Institute)
- Health Profession Students (ie. Physical Therapy)
You must also complete the following:
- Blood Borne Pathogens (print out certificate, then print this form and fill it out)
- Social Security Training (print out certificate)
- Red Flag Training (print out certificate, then print this form and fill it out)
- Emergency Contact (print and then fill out)
- Record of Volunteer Service (fill out online and print)
- Mandatory Compliance Training - held at 11am on Thursdays in Room 4223 of the 1329 Building (Call the compliance office at 352.265.8359 for questions)
- Health Assessment Form
- Immunization Record
- Copy of Driver’s License
- Health Assessment at Student Health Care Center (provide days you are available below and we will schedule an appointment for you) - You will have to pay for the assessment.
