Insurance Plan Information

To assist in your patient care experience, we will take care of filing your claims to your insurance carrier. UF Orthopaedics and Rehabilitation has arrangements with a number of insurance carriers, with some our larger agreements listed below. If you are covered by one of these companies, we ask that you pay for any co-pay amounts that apply to your policy at each visit.

If your orthopedic condition requires the completion of a disability claim, we charge a minimal fee for the completion of such documents.
