New and returning patients to the OSMI can schedule an appointment or request a patient referral in one of two manners:

A clinic representative will call you with confirmation information.

Please be sure to bring the following items to your appointment:

• Insurance information
• List of medications
• Copies of lab reports, if available
• Copies of radiographic studies (MRI's, x-rays, etc.), if available
